Social Workers are uniquely placed to assist with complex decision making. Social Workers focus on maintaining and enhancing quality of life in a range of settings to facilitate empowerment and assist people with disability, their families, and communities.
With Grace Therapy is experienced working with individuals, families, guardians, and their support systems. With Grace Therapy provides supports to people with a disability and focuses on people’s abilities and strengths.
Social Work Services
With Grace Therapy is a registered NDIS provider and staff are accredited social workers who can provide services under “Improved Daily Living” in your NDIS plan. We work with children, adults, families and carers who are seeking social work support under the NDIS.
With Grace Therapy can provide social work services through your Home Care Package. Please contact us directly or your case manager for more information about how you can use your Home Care Package to fund allied health services.
Referrals can be made by yourself, nominees, guardians, your case manager, and NDIS support coordinators, with your consent.
Our Social Work services can be funded under the National Disability Insurance Scheme (NDIS), Medicare, My Aged Care, Self funded and Work Cover.
We can assist participants with:

- Provide assessments and referrals to service providers

- Accommodation and tenancy support
- Biopsychosocial and functional assessments
- Plan review reports
- Life Transitions - leaving school, moving into a new home, becoming a parent,
- Navigating aged care, NDIS, housing, Centrelink systems
- Hospital discharge planning
